- Taken within the last six months
- Full-face, directly facing camera, neutral expression, both eyes open
- Separate photo required for entrant, spouse and each eligible child; photos must not be reused across entries/years
- If scanning a printed photo: 2x2 in (51x51 mm) scanned at 300 ppi
- No headphones; religious head covering only, full face visible
- NEW (final rule effective 2026-04-10): entry must also include passport number and a JPEG scan (max 5 MB, no PDF) of the entrant's biographic/signature passport page
